- Volunteer/Foster Application - This form needs to be completed, in its entirety, by all volunteers and fosters. Volunteers handling dogs must be at least 18 years old.
- Volunteer Expense Reimbursement Claim - Submit this form to the OBG Treasurer when you are requesting reimbursement for an approved OBG expense.
- Volunteer Charitable Contribution Claim - Use this form for tax purposes when you do not need reimbursement for an expense related to OBG. Only submit to the OBG Treasurer for expenses exceeding $250.

- Request to Add Vet - If your preferred vet is not on our current vet list, work with them to fill out this form, which will be reviewed by the OBG Board.
- Groomers List - Please use a groomer from this list, since they generously offer free or heavily discounted services to our foster dogs.

- (coming soon!) Foster Folder Documents - Every foster folder has the following documents, some of which are provided to the adopters, some of which stay with the foster home.
- Homecheck Form - This form needs to be completed after a home check. Please use one of the following options.
- Online Homecheck Form (New for 2017!) - This form can be completed in lieu of the paper copy and is automatically submitted

- Adoption Application - This form must be completed by all potential adopters. They can complete and submit using one of the following options.

- Intake Request Form - Owners fill out this online form to request that OBG take their dog.
- Surrender Form - After OBG has agreed to accept a dog, owners must sign this form to relinquish ownership and legally turn a dog over to OBG. Must be completed at/before turnover. (.pdf / MS Word)
- Dog Info Form - Upon Intake, please answer the following questions to the best of your ability and submit to doginfo@cockerspanielrescue.com
- Donation Request - Sample letter to use for requesting donated goods or services for OBG
