Oldies But Goodies Cocker Rescue

Intake Corner

The following page is for use by the intake team members.  Intake is the term by which we refer to the team of people who handle information gathering and decisions related to accepting dogs and bringing them into our rescue.  Intake is comprised of teams handling shelter requests, owner turn in (OTI) requests and intake decisions.

Intake Requests

Please fill out this form (only for volunteers!) once you have spoken to the owner, shelter or rescue POC and have gathered enough information for the team to make a decision.  If you have photos, please email to intake@cockerspanielrescue.com

The intake fax is 877-798-9806

The fax can be used to send medical records, surrender forms etc to the intake@cockerspanielrescue.com email address.  If you are expecting a fax to be sent there, please inform them so they will know what it is when it comes in.
